Tell us what you think the ADA has (or has not) done to improve life for you and your colleagues in the community in which you live. Once you’ve completed the survey, you will be entered in a drawing to win a brand new Apple iPad, iPod touch or iPod shuffle. (Three prizes will be awarded, in descending value, to the first three individuals whose names are drawn from all those eligible individuals who complete the survey. Employees or contractors of ILRU, TIRR Memorial Hermann, or UTHealth are not eligible to win.) The survey takes about 10 minutes to complete.
The survey has been sent to you because you are regarded as a leader in the disability community. When completing the survey, please answer on behalf of people with disabilities in your community. Forward the link to this survey to other individuals whom you would consider leaders in the disability community. The goal of this initiative is to report findings that are generally representative of the disability community. Personal identification is not required to complete this survey. Any personal information provided to identify entries into the prize giveaway will be separated from survey responses and maintained in a separate file.
The results of the survey will be posted online before July 26, 2010. Prizewinners will be announced online, and winners will be notified by e-mail by July 16, 2010.
